Champlain Rehab Solutions (CRS) in Ottawa is seeking an Occupational Therapist to provide high-quality, client-centered care in various settings. The successful candidate will work independently, assess clients, implement interventions, and collaborate with the care team to promote independence and uphold CRS's core values.

Key Responsibilities:

Assess and implement occupational therapy interventions based on professional guidelines

Coordinate care delivery and communicate with the client's team

Document assessment findings and treatment plans

Develop knowledge and skills to support safe client care

Required Qualifications:

Degree in Occupational Therapy from a recognized school

Member in good standing with the College of Occupational Therapists of Ontario

Valid Driver's License with access to a vehicle

Compassionate, empathetic, and self-driven

Skilled in caseload management and electronic documentation

Able to provide virtual care

Registered ADP authorizer (asset)

Actual earnings may vary based on availability and service volumesAs an Occupational Therapist at Champlain Rehab Solutions, you will be assessing and providing consultation, recommendations and interventions to a diverse group of clients in their homes using evidence-based best practice. You will ensure coordinated delivery of care through ongoing communication with the client care team regarding plans and schedules for interventions, problem solve in collaboration with the client and care team to create opportunities for more independence, and promote CRS's core beliefs and values.**Key Areas of Accountability** CRS' Occupational Therapist have the following key areas of accountability, but not limited to:

* Assesses and implements occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.
* Implement client interventions based on assessment information, client input and in conjunction with the client's integrated care plan.
* Ensure that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
* Ensure coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
* Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the organization.
* Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.

**Required Qualifications:**
