Nova Scotia Health is seeking a Pharmacist to join their team at the West Bedford Transitional Care Facility. The successful candidate will provide direct pharmaceutical care to patients, participate in medication management, patient counseling, and medication safety initiatives. The ideal candidate will have a Pharmacy degree, be registered with the Nova Scotia Pharmacy Regulator, and have at least 2 years of hospital pharmacy experience. Strong communication skills, the ability to work in a multidisciplinary team, and a commitment to professional development are essential.

Key Responsibilities:

Participate in medication management and monitoring

Provide patient counseling and education

Contribute to medication safety and quality improvement initiatives

Engage in evidence-based clinical research

Qualifications:

Graduate of a recognized School of Pharmacy

Eligible for registration with the Nova Scotia Pharmacy Regulator

Minimum 2 years of hospital pharmacy experience preferred

BLS and immunizer certification required

Strong therapeutic and medication knowledge required

Effective oral and written communication skills required

Ability to work in a multidisciplinary healthcare team required

French language proficiency preferred

Hours:

Permanent, 100% FTE, 75 Hours Bi-weekly

Compensation and Benefits:

$49.42 - $63.29 Hourly

Benefits package includes health, dental, travel, long-term disability, life insurance coverage, and a defined benefit pension plan.

About the Opportunity

Under the direct supervision of the Manager Clinical Services, the Pharmacist will participate in the provision of direct pharmaceutical care to patients primarily at the West Bedford Transitional Care Facility as a member of the interdisciplinary care team. Duties may include medication management and monitoring, preceptorship, patient counseling, liaison with outpatient providers, provision of education and responding to drug information questions. The Pharmacist participates in medication safety, quality improvement and research initiatives as appropriate.

About You

We would love to hear from you if you have the following:

Graduate of a recognized School of Pharmacy
 Eligible for the registration with the Nova Scotia Pharmacy Regulator (NSPR)
 Completion of a Hospital pharmacy residency program and/or equivalent related direct patient care experience (minimal 2 years) in a hospital setting preferred
 BLS (Basic Life Support) required
 Certification as an immunizer required
 Strong therapeutic and medication knowledge required
 Knowledge of medication &amp; patient safety concepts an asset
 Experience in evidence based clinical research an asset
 Strong teaching ability/experience required
 Experience in critical appraisal of scientific literature required
 Proven effective oral and written communication skills required
 Experience as a member of a multidisciplinary healthcare team required
 Ability to prioritize work responsibilities and to work well under time pressures required
 Demonstrated interest in professional development and lifelong learning required
 Competencies in other languages an asset, French preferred
