Description:

This job posting is for a Rapid Response Nurse position within the Winnipeg Regional Health Authority's Home Care Nursing department. The Rapid Response Nurse will provide short-term targeted interventions to medically complex/frail adults at risk for avoidable hospital readmission and/or Emergency Department visits. This role involves comprehensive assessments, medication reconciliation, chronic disease management education, review of discharge plans, and referral to community services and supports. The Rapid Response Nurse will work both in the client's home and through telephone follow-up, collaborating with the client's interprofessional team.

Experience:

  • 2 years related recent experience preferred.
  • Experience in emergency, medicine, rehabilitation, and/or community an asset.
  • Satisfactory employment record required.
  • Education:

  • Graduate of an approved Registered Nursing education program required.
  • CNA Community Health Nurses Certification or relevant applicable certification preferred.
  • Additional chronic disease management education preferred.
  • Certification/Licensure/Registration:

  • Responsible for maintaining and providing proof of registration with the College of Registered Nurses of Manitoba (CRNM).
  • Active participation in professional associations e.g. Association of Regulated Nurses of Manitoba (ARNM) preferred.
  • Cardiopulmonary Resuscitation (CPR) training requirements for this position will be in accordance with the Employer policy.
  • Valid Manitoba Class 5 driver's license required.
  • Qualifications and Skills:

  • Knowledge of various standards, acts, and protocols related to nursing care and healthcare.
  • Demonstrated effective communication skills.
  • Preference for competency in an Indigenous language and/or knowledge of Indigenous customs.
  • Critical thinking, problem-solving, and ability to adapt to changing situations.
  • Physical Requirements:

  • Moderate to heavy physical effort required.
  • Physically able to provide service in various community settings under different conditions.
  • Use of a reliable motor vehicle suitable for all environmental conditions.
  • Subject to Immunization and Tuberculin testing as per WRHA policy.
