Surrey Place is seeking a Registered Nurse for a part-time, 12-month contract position. The successful applicant will provide health care to clients with developmental disabilities and/or autism spectrum disorder as part of the TRE-ADD team. The role involves conducting health assessments, implementing treatment plans, and educating families on supporting their child/youth.

Qualifications:

Baccalaureate degree in nursing and membership with the College of Nurses of Ontario.

3 years of nursing practice experience in a community setting.

Knowledge and experience working with individuals with developmental disabilities and/or autism spectrum disorder.

Experience with case management and psychopharmacology considered assets.

Proficiency in MS Office and ability to travel within the GTA.

Position Description

Surrey Place is seeking a Registered Nurse. The successful applicant will report to the Manager, TREADD. The Registered Nurse will practice as a member of the TREADD (Treatment, Research, Education for Autism and Developmental Disabilities) Interdisciplinary team, providing health care to Surrey Place clients with developmental disabilities and/or autism spectrum disorder. Services can include direct assessments, implementation of treatments with clients, and building parent/mediator capacity for families to effectively support their child/youth. This position will also emphasize health promotion, health education and disease prevention, contributing to the excellence of health care for the child/youth.

Main Responsibilities

Conduct comprehensive health assessments for clients 18 and under with developmental disabilities and/or autism spectrum disorder, including a holistic health history and physical examination.
 Formulate, implement, and evaluate plan of care, involving clients and/or caregivers for health needs appropriate to RN scope of practice, specifically focused on developmental disabilities and/or autism spectrum disorder using evidencedbased care, and document plan of care as part of the client's service plans.
 Conduct nursing assessments and contribute to the management of episodic and stable chronic conditions commonly seen in primary health care, consulting with Surrey Place staff including the Clinical Nurse Specialist, external physicians, and/or other health practitioners.
 Work with clients, families, internal and external interprofessional teams to navigate the health care system, identify and access health and other resources in their communities.
 Act as the primary contact person for families and their child's medical team for clients requiring pharmacologic management support.
 Collaborate with the interdisciplinary team: including attending interdisciplinary meetings, document and report writing (intake, formulation, intervention and discharge), and communicating with professionals and caregivers.
 Provide nursing consultation and education to the client's team, including family, service providers, community paediatricians and family physicians on the impact of various medical conditions and medication on a child's behaviour and functioning.
 Collaborate with the interdisciplinary teams to determine a client's suitability for Surrey Place medical services and prioritize and triage clients for waiting list management.
 Act as a case manager to help clients access resources and help coordinate care of complex clients with the Surrey Place team and community partners.
 Provide longitudinal physical and mental health care to complex clients, including, intake, triage, assessment, intervention and consultation clients.
 Integrate health promotion principles into each interaction with individuals, families and groups.
 Participate in community projects, education, research and/or agency activities as a representative of Surrey Place.
 In collaboration with medical staff, build community capacity within the nursing and medical community to provide services to people with developmental disabilities and/or autism spectrum disorder.
 Maintain client records, progress notes, reports, and service plans as per Surrey Place policy and the College of Nursing of Ontario.

Qualifications

Baccalaureate degree in nursing from a recognized university or equivalent education and experience, with willingness to pursue higher education, such as a master's degree.
 Member in good standing with the College of Nurses of Ontario (in the General Class).
 3 years of nursing practice experience in a community setting or a combination of community experience and public health nursing is preferred.
 Relevant hospital nursing is considered an asset.
 Specialized nursing knowledge required to independently conduct a comprehensive health history, physical assessments, and development of a nursing management plan.
 Knowledge and experience working with individuals with developmental disabilities, and/or autism spectrum disorder, who may have a dual diagnosis, or other complex needs.
 Experience working on an interdisciplinary and/or multidisciplinary team.
 Highly developed interpersonal, communication (both written and oral), problem solving, organizational and time management skills.
 Experience with case management is considered an asset.
 Demonstrated commitment to and knowledge of communitybased health care, communitybased services and systems.
 Experience working in psychopharmacology is considered an asset.
 Knowledge of paediatric developmental and medical issues.
 Ability to travel within the GTA.
 This position may require some evening work.
 A satisfactory Police Records Check (Vulnerable Sector Screening).
 Proficient with MS Office (Word, Excel, PowerPoint and Outlook).
 Fluency in French is considered an asset.
