Description:

The Acute Care Transitions Program aims to provide seamless care for patients transitioning from hospital to home by connecting them with community-based healthcare providers, social support services, and primary care teams. We are seeking passionate Occupational Therapists to join our team and contribute to client-centered care. As part of the SE Transitions team, you will have the opportunity to work in a self-directed environment, collaborate with interdisciplinary teams, and create holistic care plans for clients.

JOB RESPONSIBILITIES:

Conducting assessments.
Formulating goals and recommendations.
Planning and implementing client-centered treatment programs.

Requirements:

Recent experience in acute/rehab or community-based rehabilitation programs preferred.
Reliable, flexible, and collaborative professional with sound knowledge of community resources.
Effective interpersonal, organizational, and communication skills.
Positive work and attendance record, ability to adapt to new computer applications.
Hybrid work approach with the ability to work remotely and in rehabilitation settings.
Vehicle, valid driver's license, and willingness to travel.

 Sound knowledge of community resources is an asset
 Effective interpersonal, organizational and communication skills
 A positive work and attendance record is essential

 This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or communitybased rehabilitation setting
 A vehicle and valid driver's license and the ability to travel
