Key Responsibilities:

Coordinate, supervise, and coach Community Support Workers

Conduct professional assessments of patients in various settings

Develop and maintain effective Care Plans

Set and work towards measurable goals with patients

Provide workplace Health and Safety guidance

Collaborate with interdisciplinary care teams

Perform other duties as needed

Qualifications:

Master's Degree in Occupational Therapy

Registration with the College of Occupational Therapists of Ontario

Strong computer skills

Excellent assessment and treatment abilities

Experience providing therapeutic support to clients

Ability to promote client independence

Clear Background and Vulnerable Sector Check

This position offers a competitive compensation grid, with progression based on hours worked and rolespecific criteria. The OT per visit range is $72.31 to $84.32 and the hourly range is $36.15 to $42.15. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate's knowledge, skills, qualifications, and relevant experience.What The Role Involves Our Occupational Therapists provide a critical role in providing high quality patient care

Provides coordination, supervision, and coaching to the Community Support Workers in the field
 Professional assessment of patients in the community and in their homes
 Develops and maintains an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome
 Works with the patient on appropriate timespecific, measurable goals and outcomes in collaboration with the care team
 Works independently and as an integral member of an interdisciplinary care team
 Provides active guidance for workplace Health and Safety
 Performs other duties as required

What You Bring

Master's Degree (or equivalent) degree in Occupational Therapy from a recognized university
 Registration with the College of Occupational Therapists of Ontario
 Proven computer skills, including email, MS Word and Outlook, work scheduling systems
 Strong assessment skills and the ability to participate in effective treatment programs
 Provide oneonone therapeutic support with clients
 Promotes independence of the client in the home and in the community
 A reliable vehicle for some regular travel
 Excellent English verbal and written communication skills
 French language skills are an asset
