If hired as a Personal Support Worker - Rehab, your responsibilities would include:

Supporting Residents with rehabilitative care activities

Collaborating with the Rehab Lead and team to enhance the Home's Rehab Program

Assisting with documentation and evaluation of rehab outcomes

Mentoring on-floor staff in Nursing Rehab practices

Participating in education and coaching related to rehabilitation programs

Using problem-solving skills to support complex rehab situations

Communicating effectively with Residents and families

Applying principles of Continuous Quality Improvement

Promoting Resident-centered care and supporting team goals

Assisting the team as needed

Qualifications:

To qualify for this role, you must have:

PSW or HCA Certification

Restorative/Rehabilitation Certification

Physical ability to transfer Residents and equipment

Recent VSS check and TB 2 Step update

 Worked closely with the Rehab Lead and interdisciplinary team to implement, sustain, and improve the Home's Rehab Program
 Assisted with documentation and evaluation of rehab outcomes, ensuring information was accurate and up to date
 Mentored and supported onfloor staff in Nursing Rehab practices and principles
 Participated in education, inservices, and coaching related to rehabilitation programs and safe mobility
 Used problemsolving skills to support complex rehab situations while maintaining Resident safety and dignity
 Communicated effectively with Residents and families regarding rehab approaches and progress
 Applied principles of Continuous Quality Improvement to daily practice
 Promoted Residentcentred care while supporting both team and organizational goals

Qualifications To be hired into this role, you must have:

Personal Support Worker (PSW) or Health Care Aide (HCA) Certification
 Restorative / Rehabilitation Certification
 Physically able to transfer/transport Residents, tables, chairs, and equipment
 Vulnerable Sector Screening (VSS) check completed within the past 6 months
 TB 2 Step updated within the last year
