In this role, you will develop care plans, provide direct OT interventions, and collaborate with clients and families to ensure services are tailored to their needs. The position requires travel to provide coverage in Didsbury, Airdrie, and surrounding rural areas.

As an OT, you will focus on occupational performance to meet the needs of individuals, groups, or communities. You will engage clients in meaningful occupations, support them in achieving functional abilities and independence, and collaborate with various stakeholders to advance services. Additionally, you will provide comprehensive services, including assessment, treatment, education, and case management.

Reporting to the designated Care Manager within Home Care, the OT Case ManagerInterventionist works as a member of an interprofessional health care team; within the full scope of their professional practice and is responsible for assessing, planning, organizing, reviewing and evaluating the care needs of Short term acute, Acute, Wellness and Rehabilitation clients and their caregivers/families requiring community based services; fosters and promotes continuity of care and cooperative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involved in the provision of services. There is an expectation to be a mobile worker as this role requires travel. This position provides coverage to clients in both Didsbury &amp; Airdrie and surrounding Rural areas. You will be responsible to operate a laptop computer and mobile phone, and manual handling (lifting, lowering, carrying, pushing and pulling) is a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job.

As an Occupational Therapist (OT), you will provide evidenceinformed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including selfcare, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, selfmanagement, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students.
