We are looking for an Occupational Therapist to join our Home and Community Care (HCC) team to provide professional support services in the homes of clients of all ages &amp; stages of life. You will administer assessments &amp; implement interventions in clinical areas such as functional transfers &amp; mobility, home accessibility, pressure &amp; wound management, cognitive screening &amp; assessment, wheelchair &amp; seating assessment, eating/feeding &amp; swallowing assessments, etc. Restorative &amp; compensatory Interventions may be used to support clients to achieve optimal functional abilities &amp; independence related to their cognitive, affective, spiritual &amp; physical status. As an Occupational Therapist (OT), you will provide evidenceinformed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including selfcare, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, selfmanagement, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 2530 lbs. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a handson assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Outlook and the Internet.
