Stem Health is reimagining the primary care experience with a focus on building relationships and providing exceptional service. They are seeking a Care Coordinator and Client Advocate to manage care coordination, client advocacy, care planning, insurance navigation, and documentation within their membership-based clinic.
Key Responsibilities
Care coordination and referral management
Client advocacy and relationship management
Care planning and interdisciplinary support
Insurance, referral, and administrative navigation
Documentation and clinical workflow
Qualifications & Experience
Registered Nurse (RN) or Registered Practical Nurse (RPN) preferred
Bachelor of Science in Nursing or related field
Minimum 3 years of care coordination experience
Strong knowledge of Ontario's healthcare system
Excellent communication and organizational skills
Proficiency with EHR/EMR systems
Commitment to member privacy and ethical health navigation
Additional training in chronic disease management or care navigation is an asset
Bilingualism is an asset
