Key Responsibilities:

Qualifications:

Nursing diploma from an accredited program; Bachelor's degree preferred.

Current license as a Registered Nurse (RN) or Registered Practical Nurse (RPN) in Ontario.

5-10 years of progressive healthcare experience.

Proven ability to build referral relationships and navigate the Canadian healthcare system.

Strong knowledge of medical terminology and patient care best practices.

Experience in discharge planning and team-based care environments.

Proficiency in Microsoft Office and strong communication skills.

Reporting to the Area Director, Transitional Care Programs, this position is ideal for a healthcare leader who values collaboration, continuous improvement, and compassionate care.What You'll DoSupervise and support frontline staff to ensure care delivery aligns with provincial legislation, professional standards of practice, and Bayshore policies.   
 Oversee clinical operations, ensuring patient assessments, care plans, treatments, and followups are completed accurately and efficiently.   
 Collaborate with team to implement patient satisfaction initiatives and ensure timely resolution and documentation of patient concerns.   
 Liaise with hospital partners, funders, and government programs to support coordinated care and effective transitions.   
 Lead recruitment, orientation, training, performance management, and disciplinary processes in collaboration with Human Resources.   
 Assist in the implementation of Modified Work Programs for staff returning from leave or requiring accommodation.   
Interpret and apply policies and procedures relevant to the Transitional Care Unit to ensure consistency and compliance.   
 Ensure adherence to Occupational Health and Safety standards, including conducting inspections, maintaining safety committees, and reporting incidents.   
 Maintain compliance with Public Health standards, Ministry of Health directives, and Bayshore's Infection Prevention and Control protocols.
