Unity Health Toronto is seeking a casual Clinical Pharmacist to provide direct patient care in managing drug therapy for medical-type inpatient units. The role involves reviewing medication orders, resolving drug therapy problems, providing patient education, and ensuring the safe and cost-effective use of drugs throughout the hospital. The position also includes participating in educational and research programs, developing medication protocols, and precepting pharmacy students.

Key Responsibilities:

Preceptorship of pharmacy students for on-site clinical education

Qualifications:

Graduation from recognized pharmacy degree program

Licensure with the Ontario College of Pharmacists

Hospital Pharmacy residency an asset

Previous experience with EPIC an asset

Previous hospital experience in direct patient care in a team environment an asset

Self-motivated team player, reliable, cooperative, and patient-focused

Excellent interpersonal and communication skills

Excellent attendance record

Role: Clinical Pharmacist (Casual)

To provide direct patient care in the management of drug therapy to achieve positive patient health outcomes, primarily for patients of a medicaltype inpatient unit, and coverage to other area as needed. Direct patient care focuses on review of medication orders; selection and assessment of patient´s requiring pharmaceutical care; identifying, resolving and preventing drug therapy problems for a subset of patients; patient education; patient continuity of care; and the provision of drug information.

To facilitate and promote accurate, safe, appropriate and cost effective use and distribution of drugs throughout the hospital, including weekend, evening and on call rotations.

To support and participate in educational and research programs sponsored by the pharmacy discipline or hospital.

To participate in the development, revision, and implementation of standards, protocols, procedures, guidelines pertaining to drug therapy and pharmacy practice.
