* **++For Registered Nurses (RN)-++** This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. **The RN per general visit range is $ 27.00 to $39.87** and **the RN hourly range is $31.15 to $46.00.** Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate's knowledge, skills, qualifications, and relevant experience.
* **++For Registered Practical Nurses (RPN)++** - This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. **The RPN per general visit range is $ 22.97 to $28.62** and **the RPN hourly range is $26.38 to $32.40.** Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate's knowledge, skills, qualifications, and relevant experience.

What The Role Involves
- ---------------------

* Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
* Working with patients (**both in-person** **and** **virtually**) to provide exceptional nursing services
* Providing services in a variety of settings which may include in-home, clinic, retirement home or school
* Communicating and liaising with patients, family members and others within the interdisciplinary care team
* Performing other clinical duties as required

What You Bring

* Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
* Current College of Nurses of Ontario (CNO) registration in good standing
* A strong desire and commitment to making a difference in the lives of our patients
* Exceptional interpersonal skills
* A valid driver's license and vehicle as regular travel is required
* Excellent English verbal and written communication skills
* CPR Certification
* Current immunization record including 2 step TB test
