Nova Scotia Health is seeking a Health Care Social Worker for a temporary full-time long assignment position at Cape Breton Regional Hospital. The successful candidate will be part of an interdisciplinary team providing holistic care across the primary to quaternary care continuum. Social Workers focus on the relationships between individuals and their environments, emphasizing psychosocial, emotional, and spiritual well-being. The role involves engagement, assessment, planning, intervention, and evaluation based on social work theory and practice.

Key Responsibilities:

Master of Social Work Degree from an accredited university

Registered or eligible for registration with the Nova Scotia College of Social Workers

Minimum of two years clinical experience

Extensive knowledge of community resources

Ability to work independently and with multidisciplinary teams

Coordinate care plans for hospital discharge and transition back to the community

Apply evidence-based practice to patient services

Competencies in other languages, French preferred

Qualifications:

Full-time long assignment / 75 Hours Bi-weekly

Starting as soon as possible and ending October 25, 2027

Shifts may include evenings, weekends, and holidays

Compensation and Benefits:

$40.18 - $54.14 Hourly (Developmental scale for less than 2 years experience: $35.83 - $46.37 hourly). Successful candidates may be eligible for health, dental, travel, long-term disability, life insurance coverage, and a defined benefit pension plan.

Social Workers are primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation. Social Workers build upon the strengths of patients/clients by focusing on existing coping strategies and problemsolving skills, past successes, and personal resources.About You We would love to hear from you if you have the following:

Master of Social Work Degree from an accredited university

If there are no candidates who meet the above qualification, those enrolled in a master's program could be considered
 Registered or eligible for registration with the Nova Scotia College of Social Workers (NSCSW)
 Minimum of two years clinical experience required
 Evidence of extensive knowledge of community resources
 Evidence of ability to work independently with a multidisciplinary team as well as with both hospital and communitybased services / agencies
 Evidence of ability to coordinate / develop / document and implement care plan for hospital discharge and safe transition back to the community or institutional setting
 Demonstrated use of innovative problem solving and application of evidencebased practice to patient services
 Knowledge of Joint Discharge Operations Committee and its purpose
 Competencies in other languages an asset, French preferred

Minimum two (2) years of experience required. If not available, candidates with less than two (2) years of experience may be considered for the selection process.
