Nova Scotia Health is seeking a Health Care Social Worker to join their team at Cape Breton Regional Hospital in the Eastern Zone. This is a temporary full-time position with a long assignment. The successful candidate will be part of a passionate team providing high-quality inpatient and outpatient services, as well as continuing care, primary health care, public health, and mental health and addictions.

Key Responsibilities:

  • Collaborate with the interdisciplinary health care team to provide holistic care
  • Assess, plan, intervene, and evaluate patient care plans
  • Coordinate and implement care plans for hospital discharge and transition back to the community
  • Utilize innovative problem-solving and evidence-based practice in patient services
  • Demonstrate knowledge of community resources and ability to work independently and with a team
  • Participate in Joint Discharge Operations Committee
  • Qualifications:

  • Master of Social Work Degree from an accredited university
  • Registered or eligible for registration with the Nova Scotia College of Social Workers
  • Minimum of two years clinical experience
  • Knowledge of community resources and ability to work independently and with a team
  • Competencies in other languages an asset, French preferred
  • Hours:

  • Full-time long assignment, 75 hours bi-weekly
  • Shifts may include evenings, weekends, and holidays
  • Compensation and Benefits:

  • Hourly rate ranging from $40.18 to $54.14 for experienced Health Care Social Workers
  • Hourly rate ranging from $35.83 to $46.37 for those with less than two years of experience
  • Benefits package available including health, dental, travel, disability, and life insurance coverage
  • Defined benefit pension plan
