Nova Scotia Health is seeking a Health Care Social Worker to join their team at Cape Breton Regional Hospital in the Eastern Zone. This is a temporary full-time position with a long assignment. The successful candidate will be part of a passionate team providing high-quality inpatient and outpatient services, as well as continuing care, primary health care, public health, and mental health and addictions.
Key Responsibilities:
Collaborate with the interdisciplinary health care team to provide holistic care
Assess, plan, intervene, and evaluate patient care plans
Coordinate and implement care plans for hospital discharge and transition back to the community
Utilize innovative problem-solving and evidence-based practice in patient services
Demonstrate knowledge of community resources and ability to work independently and with a team
Participate in Joint Discharge Operations Committee
Qualifications:
Master of Social Work Degree from an accredited university
Registered or eligible for registration with the Nova Scotia College of Social Workers
Minimum of two years clinical experience
Knowledge of community resources and ability to work independently and with a team
Competencies in other languages an asset, French preferred
Hours:
Full-time long assignment, 75 hours bi-weekly
Shifts may include evenings, weekends, and holidays
Compensation and Benefits:
Hourly rate ranging from $40.18 to $54.14 for experienced Health Care Social Workers
Hourly rate ranging from $35.83 to $46.37 for those with less than two years of experience
