Key Responsibilities:

Deliver comprehensive primary health care services within the Primary Health Care clinic

Provide care through same-day access and/or a rostered client model

Deliver culturally responsive care independently and in collaboration with an interprofessional team

Manage a diverse range of health conditions and complex needs of the populations served

Contribute to the development, implementation, monitoring, and evaluation of programs and services

Qualifications:

Current extended class certificate (NP-PHC) and unrestricted registration with the College of Nurses of Ontario

Minimum two years' experience as a Nurse Practitioner in a primary care setting

Fluency in English required, working knowledge of other languages is an asset

Passion for working with high-risk populations and people who use drugs

Knowledge of primary care, health promotion, and disease prevention

The NP practices to their full scope, providing care through sameday access and/or a rostered client model to meet program needs. They deliver culturally responsive care both independently and in collaboration with an interprofessional team. The successful candidate will demonstrate the ability to work autonomously and collaboratively, with a strong commitment to culturally safe, equitable, and clientcentred care. This position requires availability to provide service one evening per week, until 8:00 p.m.The NP is responsible for delivering primary health care services in accordance with the Standards of Practice for Registered Nurses in the Extended Class and the Expanded Nursing Services for Patients Act. This includes health promotion, counselling, disease prevention, diagnosis, treatment, and referrals to specialists or diagnostic testing. Providing clinical services requires the NP to manage a diverse range of health conditions while addressing the complex needs of the populations served.The NP plays an active role in the development, implementation, monitoring, and evaluation of programs and services, ensuring accessible and highquality care for individuals with diverse health, social, cultural, and economic needs. Current extended class certificate (NPPHC) and unrestricted registration with the College of Nurses of Ontario
 Knowledge of the ODSP application process
 Experience with using Practice Solutions EMR is an asset
 Certification in CPR (BLSC)

Minimum two years' experience as a Nurse Practitioner working in a primary care setting
 Strong clinical skills and diagnostic abilities for clients across the life span
 Previous experience working with culturally diverse and marginalized populations
 Interest in providing outreach services

◈ Required Language

Fluency in English required.
 Working knowledge of other languages is an asset.

The Nurse Practitioner (NP) provides a comprehensive range of services, including triage, physical assessment, diagnosis, treatment, counselling, health promotion, and preventative care, tailored to clients with diverse health, social, cultural, and economic needs. Working to the full scope of practice within the Walkin Clinic and/or a Rostered practice, the NP will be expected to function both independently and as part of an interprofessional team, delivering care in a culturally sensitive manner. The successful candidate may also be required to provide services during one evening per week until 8:00 p.m.
