The Leeds & Grenville Community Family Health Team (LGCFHT) is seeking a Nurse Practitioner to join their team on a temporary, full-time basis. The NP will work collaboratively with other healthcare professionals to provide primary care services to patients, focusing on health promotion, disease prevention, and illness management.
Key Responsibilities:
Engage with patients to manage their personalized health care needs
Work to the full scope of the NP role as identified by the College of Nurses of Ontario
Conduct independent health care assessments and patient management
Collaborate with inter-professional teams
Demonstrate leadership, communication skills, and problem-solving abilities
Qualifications:
Current registration as Registered Nurse in the Extended Class with the College of Nurses of Ontario (CNO)
Completion of a recognized Primary Care Nurse Practitioner program: NP-PHC
Masters of Nursing or related graduate degree with a clinical focus is an asset
Completion of an accredited controlled substance course according to CNO standards
Minimum of 3-5 years NP experience, with previous primary health care experience
