Description:
The Parkinson and Movement Disorders Program (PMDP) is seeking a collaborative Nurse Practitioner (NP) to join an established interdisciplinary specialty team providing care to adults living with Parkinson disease and other movement disorders. The NP functions within a collaborative interdisciplinary model of care, providing specialized assessment, diagnosis, therapeutic management, prescribing, patient education, chronic disease management, and care coordination. Working with Movement Disorder Neurologists and the interdisciplinary team, the NP provides advanced nursing care across the continuum of Parkinson and movement disorder care.
The role emphasizes continuity of care, symptom management, follow-up, timely access, and coordination with primary care, Home Care, allied health, and community services to optimize patient outcomes and quality of life. Specialist neurologists remain integral members of the care team, providing diagnosis, complex treatment direction, and consultation according to established collaborative care pathways.
As a Nurse Practitioner (NP), you will provide comprehensive health services to individuals across their lifespan. You will integrate clinical skills associated with nursing and medicine to assess, diagnose, order and interpret diagnostic tests, prescribe treatment, and perform procedures to meet patient health needs. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership, Education, and Research to promote care quality and inform healthcare systems.
