Description:

Integrated Home Care is seeking an Occupational Therapist (OT) to join their team in a role that involves developing care plans, providing direct OT interventions, and collaborating with clients and families to ensure ongoing services meet their needs. The OT Case Manager-Interventionist will work within a strengths-based Client Centered Case Management practice framework, focusing on Short term acute, Acute, Wellness, and Rehabilitation clients in the Chestermere and Airdrie Geographical Areas.

As an OT, you will provide evidence-informed services to enable meaningful and chosen occupations for individuals, groups, or communities. This includes assessing, planning, organizing, reviewing, and evaluating care needs, fostering continuity of care, and liaising with primary care physicians and other healthcare providers. The role requires travel, manual handling, and the use of a laptop computer and mobile phone.

Responsibilities include triage, screening, consultation, assessment, treatment, prevention, education, case management, referral, transition support, and collaboration with stakeholders. You will engage clients and families in all aspects of care, participate in interprofessional teams, and contribute to quality improvement and professional development.

**Required Qualifications:**

- Completion of a bachelor's degree in Occupational Therapy from an accredited university program

- Registered with the Alberta College of Occupational Therapists (ACOT)

**Additional Required Qualifications:**

- Three years of recent home care or acute care experience in the last five years

- Proficiency in computer use

- Ability to work to full scope of practice and use critical thinking skills

**Preferred Qualifications:**

- Experience in acute and long-term care with a focus on chronic disease management

- Experience in providing case management and interventionist services

- AADL Authorizer certification
