Assisted Living Alberta is seeking an Occupational Therapist (OT) to join the Children's Home Care (CHC) program in Edmonton. The OT will act as both a Case Manager and a direct care provider, working with children from birth to 18 years old with various medical and developmental needs. The role involves assessing, organizing, and communicating with clients and families to develop care plans and provide support services.

As an OT, you will provide evidence-informed services focusing on occupational performance to meet individual, group, or community needs. You will engage with clients to enable meaningful occupations, support functional abilities, and promote independence. The role includes comprehensive services such as assessment, goal setting, treatment, education, and collaboration with stakeholders. You will also participate in interprofessional teams to improve services and support clients.

Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Specialty Programs. The Specialty Programs portfolio includes Children's Home Care (CHC). CHC has an opportunity available for an Occupational Therapist (OT). As an OT for CHC, you will act as both a Case Manager (CM) and a direct care provider, using a familycentered approach to practice, coaching, assessing, organizing, and communicating. The case management duties include ongoing assessment and identification of the unmet needs of the child/family. In collaboration with the client/family and other partners, and using appropriate internal and external resources, the CM develops a care plan for supports and services that is modified, based on the client's response to the interventions.

Description:

As an Occupational Therapist (OT), you will provide evidenceinformed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including selfcare, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, selfmanagement, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients.
