About The Role

Location: Remote - Canada based only

Title: Care Coordinator

We're seeking a full-time remote Care Coordinator to be the initial contact for patients at Sailor Health. In this role, you will guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported throughout.

This position requires a blend of patient support, care coordination, and emotional guidance. You will assist patients in finding the right therapist and provide reassurance and trust throughout their journey into care.

This role is ideal for individuals interested in mental health and healthcare access, as you will be the first emotional support for those seeking care.

It's a perfect fit for someone who thrives at the intersection of empathy and execution, and who finds meaning in turning a moment of vulnerability into one of reassurance and trust.

This role is especially wellsuited for someone with an interest in mental health and healthcare access  since you'll often be the first emotional point of contact for individuals seeking care.

 Highly organized and detailoriented. You can juggle multiple patients at different stages in the process and never let things slip through the cracks.
 Techcomfortable. You're comfortable learning tools like Google Workspace, EHR platforms (e.g., Healthie), or CRMs like HubSpot, and pick up new software quickly.
 Missionaligned. You care deeply about improving care for older adults and are excited to play a key role in someone's first step toward better mental health.
