CarePartners is seeking a Personal Support Manager to join their Leadership Team. This full-time position involves coordinating and supervising the delivery of Community Health Services. The role requires participation in Branch Manager On-Call, including evenings and weekends on a rotational basis.

What The Role Involves:
- Providing supervision and coaching to Community Support Workers
- Developing and maintaining effective Care Plans
- Keeping abreast of policies and best practices
- Promoting Community Health Services
- Guiding workplace Health and Safety
- Performing other duties as required
What You Bring:
- RN or RPN degree
- 3-5 years of related experience
- Registration with the College of Nurses of Ontario
- Computer skills
- Case management knowledge
- Leadership and people management skills
- CPR Certification
- Clear Background and Vulnerable Sector Check
