CarePartners is seeking a Personal Support Manager to join their Leadership Team. This Full-Time position involves coordinating and supervising the delivery of Community Health Services. The role requires participation in Branch Manager On-Call rotations, including evenings and weekends.

What The Role Involves:
- Coordinating, supervising, and coaching Community Support Workers
- Developing and maintaining effective Care Plans
- Keeping abreast of regulations and best practices
- Promoting and marketing Community Health Services
- Providing guidance for workplace Health and Safety
- Performing other duties as required
What You Bring:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- 3 - 5 years of job-related experience or equivalent education/experience
- Registration with the College of Nurses of Ontario (CNO)
- Experience in community healthcare environment and formal leadership role considered assets
- Proven computer skills and knowledge of case management and nursing processes
- Vehicle required for travel
- Leadership and people management skills
- Excellent English verbal and written communication skills
- French language skills and Current CPR Certification considered assets
- Clear Background and Vulnerable Sector Check
