St. Joseph's Home Care is seeking a Personal Support Worker for their Assisted Living Programs to provide quality and compassionate care to clients in their own home/apartment units. The PSW-HALP will work closely with the Manager and Supervisor of Assisted Living Programs to ensure client-centered care and the execution of daily tasks. Clients are typically individuals who have been discharged from the hospital and need short-term care.

Core Duties And Responsibilities

Assisting clients with daily personal care activities, such as bathing, dressing, grooming, and personal hygiene tasks

Assisting with housekeeping activities, laundry services, and meal preparation

Supporting clients in maintaining physical health and safety through safety checks, medication reminders, and mobility assistance

Providing emotional and social support to clients and their families

Following established Assisted Living procedures and care plans

QUALIFICATIONS

PSW Certificate from an accredited training institution

1-3 years of PSW experience

Current First Aid and/or CPR Certificate

Training in gentle persuasive/non-violent crisis prevention

Access to reliable transportation, driver's license, and vehicle insurance

Vulnerable sector police check and 2-step TB test at the time of hire

Personal Support Worker POSITION SUMMARY: The Personal Support Workers  Assisted Living Programs (PSWHALP) enhance the quality of life of clients by providing quality and compassionate care in the form of personal care assistance, medication reminders, physical and cognitive support and home keeping assistance. PSWHALP assists in enabling individuals to continue living independently in a safe and healthy manner in their own home/apartment units, within the various Halton Assisted Living program locations and the surrounding community.

The PSWHALP works cooperatively with the Manager Assisted Living Programs (MHALP) as well as the Supervisor  Assisted Living Programs (SHALP) in providing quality clientcentered care, and in the effective execution of daily tasks. PSWHALP adheres to daily care schedules and overall care plans.

HALP clients are typically individuals who have been discharged from hospital but who are unable to return to their prehospital living arrangements. These clients are in process of having long term care needs assessed, are generally medically stable, can be of any adult age group / elderly age group, and may require mobility assistance. Client stays are usually short term in length, and they may require a care plan.

Core Duties And Responsibilities

Assisting clients with daily personal care activities, in keeping with their individual care plan

Bathing

Dressing/undressing

Personal hair care

Shaving/grooming

Mouth care

Preventative skin care

Other personal hygiene tasks
 Assisting clients with daily housekeeping activities, in keeping with their individual care plan

Light apartment housekeeping (sweeping, tidying, dishes etc.)

Laundry services

Light meal preparation / safe food handling
 Assisting clients in daily activities which maintain adequate levels of physical health and safety, in keeping with their individual care plan

Performing safety wellness checks

Medication reminders

Living area visual scans

Engaging clients in positive/social conversations

Encouraging clients to direct and/or participate in their own care

Assisting clients with general mobility needs (walking, movement, positioning etc.) in and around their home and the program location

Encouraging participation in HALS social and/or recreational activities

Assisting with basic range of motion exercises where required by the care plan

Demonstrating sensitivity and respect to clients who may be experiencing physical health challenges

Providing emotional and social support to both clients and their family members

Demonstrating a positive attitude and approach towards clients as unique individuals
 Performs daily work in keeping with the established Assisted Living procedures, processes and expectations

Answers call bells in a safe and timely manner

Observes and records client information/comments/issues/behaviours/changes on the 24Hour Shift Report

Follows daily client care schedules as determined by the Manager or Supervisor

Maintains a clean and organized work environment

Reports critical client care information/issues to Manager or Supervisor right away

Understands and follows individual client care plans, seeks clarification from the Manager or Supervisor where care plan in unclear

Participating in team/group meetings

Other duties as requested or required

QUALIFICATIONS

PSW Certificate from an accredited training institution, required
 13 years PSW experience, preferred
 Member of the PSW Registry, preferred (may be required under some contracts)
 Current First Aid and/or CPR Certificate, required
 Training in gentle persuasive/nonviolent crisis prevention, preferred
 Training and previous work experience in Medication Reminders, preferred
 Previous experience working in a Assisted Living environment, preferred
 Access to reliable transportation, driver's license and vehicle insurance, required
 Vulnerable sector police check at time of hire, annual declaration, required
 2 step TB test at time of hire, required
