VON Canada in Trenton, Ontario is seeking a Personal Support Worker (PSW) for a permanent, part-time position. The PSW will provide personal support and homemaking services to clients and families within VON policies and standards. The role involves collaborating with the health care team to ensure clients achieve optimal independence and well-being.
Key Responsibilities:
Observing and evaluating client and family situations to determine care plan changes
Contributing to care plan development under the direction of the Program Manager
Implementing care plan changes for safety and financial reasons
Reporting changes in client needs to supervisors
Providing personal support tasks and care as directed
Completing daily tasks and forms for effective service operation
Assessing communication patterns and ability to relate
Complying with health and safety regulations and VON policies
Promoting VON's goals and values
Working collaboratively with other staff
Education, Designations and Experience:
High school diploma
CCA Certification required in Nova Scotia
Certificate in Home Support Worker or equivalent
Current First Aid or CPR certification
Post-secondary degree or diploma an asset
Experience in household management and working with elderly or ill individuals
Skill Requirements:

Awareness of community resources
Ability to work in a team or independently
Capacity to adhere to strict confidentiality

Access to a personal vehicle and driver's license
