The Registered Nurse - Case Manager role involves directing client care, requiring strong communication skills, critical thinking, and organization abilities. Responsibilities include assessing client needs, planning and delivering care, and coordinating services within professional standards.
Key Responsibilities:
Provide a wide variety of nursing services to patients, families, communities, and populations
Utilize nursing processes through critical thinking, problem-solving, and decision-making
Teach, counsel, and advocate for patients and their families
Support an integrated and holistic approach to patient care and health promotion
Reflect the shared vision and values of AHS in providing safe, quality patient-centered care
Qualifications:
Completion of an accredited nursing education program
Active or eligible for registration with the College of Registered Nurses of Alberta
Current Basic Cardiac Life Support - Health Care Provider certification
Ability to be on call and report to the site/client's home within 30-45 minutes
Training in case management principles is an asset
Knowledge of the continuing care sector and various levels of care is preferred
Experience in wound care and Connect Care is an asset
