Description:

The Palliative Home Care Program aims to provide exceptional palliative care to individuals and families in the community. The Palliative Registered Nurse: Case Manager works within an inter-professional care team under the Care Manager of the program. Responsibilities include case management, direct nursing care, and collaboration with the care team and primary care physician. This position is on-site and does not allow for remote work.

As a Registered Nurse, you will deliver a range of nursing services to patients, families, and communities while ensuring their safety and well-being. You will use critical thinking, problem-solving, and decision-making skills to develop care plans, coordinate interventions, and advocate for patients. Leadership in patient care, health promotion, and maintenance is essential. Provisional Permit holders will receive a specific hourly rate, which will be adjusted upon obtaining a full practice permit.

  • Qualifications:

  • Completion of an accredited nursing education program
  • Active or eligible for registration with the College of Registered Nurses of Alberta
  • Current Basic Cardiac Life Support - Health Care Provider certification
  • Three years of community or acute medical/oncology nursing experience within the past five years
  • Recent Palliative Care experience
  • Strong interpersonal and communication skills
  • Ability to work in an inter-professional team
  • Proficient in Microsoft Office software
  • Physically able to perform all aspects of the position
  • Orientation to the role is full-time for 6 consecutive weeks
  • Preferred Qualifications:

  • One year of Palliative Home Care Experience in the last two to three years
  • Baccalaureate or advanced degree in Nursing
  • CNA certification in Hospice Palliative Care
  • Case Management Certification
