Humber River Health is seeking a Part-time Social Worker to join their team at the Wilson Site. The successful candidate will provide compassionate care to a variety of patient populations, including acute medical, ICU, Surgical, Emergency, and Geriatric Patients. Responsibilities include psychosocial assessment, crisis intervention, discharge planning, and counselling for patients and families. The Social Worker will act as a liaison, assist with community resources, and manage patient flow within the acute and community sectors. Additionally, they will provide clinical supervision to social work students and participate in reflective practice.

Qualifications for this position include an MSW from an accredited school of Social Work, current membership with OASW and OCCSWSSW, and experience in the acute and/or sub-acute sector system. The ideal candidate will have strong organizational skills, computer proficiency, and excellent communication abilities. They must demonstrate the ability to work independently and in a team environment, as well as cross-cultural awareness and sensitivity.
