Description:

Integrated Home Care uses a strengths-based Client Centered Case Management practice framework. You will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals. In collaboration with the client/family and interdisciplinary team, you will ensure services are provided and modified based on the client's response to interventions. Reporting to the Home Care Manager, you will coordinate and provide quality care to clients in the community, offering assessments, case coordination, and direct care. The successful incumbent will demonstrate proficiency in personal and professional accountability, systems thinking, creative thinking, communication, relationship-building, time management, interpersonal skills, and intermediate computer skills.

As a Social Worker II, you will be part of an interdisciplinary team providing social work services to patients and families. Your services may include psychosocial assessment, resource-based interventions, individual, group, and family treatment, risk screening, counseling, case management, transition planning, and prevention activities.
