Alberta Kidney Care North (AKC-N) is seeking a Renal Social Worker to provide psychosocial support to individuals with kidney disease in the Grande Prairie hemodialysis unit. The role involves conducting comprehensive psychosocial assessments, adjustment counseling, resource navigation, advocacy, collaboration, education, documentation, and interdisciplinary teamwork. The position is eligible for the Remote Retention Allowance, with a competitive rate of pay and an annual maximum of $3,000.

As a Social Worker II, you will be part of an interdisciplinary team, offering services such as psychosocial assessment, resource-based interventions, individual, group, and family treatment, risk screening, counseling, case management, transition or discharge planning, and prevention, promotion, and public awareness activities.

Required Qualifications:

Bachelor's degree in Social Work

1-2 years' experience in renal acute care/ambulatory setting or working with patients facing chronic illnesses

Cultural awareness and sensitivity

Strong interpersonal skills

Knowledge of the healthcare system and relevant legislation

Good understanding of health promotion and social determinants of health

Current Basic Life Support (BLS-HCP) certification

Clear communication skills

Preferred Qualifications:

Proficiency with Microsoft Office and Connect Care

Master's Degree in Social Work

Authorized by ACSW to provide Psychosocial Interventions

Training in suicide prevention, risk assessment, and Non-Violent Crisis Intervention

In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum 12 years' experience working as an RSW in renal acute care/ambulatory setting or recent experience working with patients facing chronic and/or lifethreatening illnesses required. Cultural awareness and sensitivity, and experience serving vulnerable populations are essential. The candidate will hold working knowledge of the health care system, community resources, and relevant legislation and decisionmaking (Personal Directives Act, Mental Health Act). The candidate will have demonstrated strong interpersonal skills to work collaboratively and effectively with teams, patients and their families in a fastpaced environment. Ability to initiate work and make sound professional decisions using critical thinking and problemsolving skills, in a complex multidisciplinary environment, and seek support and guidance from leadership as needed. Good understanding of health promotion and social determinants of health. Strong time management skills are required to prioritize and complete assigned tasks. Current Basic Life Support (BLSHCP) certification is required. Demonstrated clarity in your communication, organization and consistency of ideas, both verbally and written, including clinical documentation, is required.

Preferred Qualifications:

Proficiency with Microsoft office including Word and Outlook are preferred. Experience using Connect Care preferred. Master's Degree is an asset. Authorized by ACSW to provide Psychosocial Interventions preferred. Training in suicide prevention; risk assessment; and/or NonViolent Crisis Intervention preferred.
