CarePartners is a leading home health care provider in Ontario, offering a range of services to patients of all ages. They are currently seeking Team Coordinators to facilitate communication between patients, families, and staff, ensuring efficient scheduling and administrative tasks are completed.
Key Responsibilities:
Act as a liaison between patients, caregivers, staff, and other stakeholders
Coordinate service schedules and communicate changes to staff
Input information into databases and produce reports as needed
Handle inbound calls and emails related to services
Perform general administrative duties

Commitment to patient care
Post-secondary education preferred, with one year of relevant experience
Strong interpersonal and customer service skills
Proficiency in Microsoft Office
Ability to work independently and as part of a team
Clear background check
Qualifications:
Experience in healthcare field an asset
Availability for non-traditional hours, including evenings and weekends
