CarePartners is a leading home health care provider in Ontario, offering a range of services to patients of all ages. They are currently seeking Team Coordinators to facilitate communication between patients, families, and staff, ensuring efficient scheduling and administrative functions.

Part-time positions are available in the Peterborough office, with defined shifts including weekends. The role offers competitive compensation and benefits, as well as opportunities for continuing education and volunteer work.

Key Responsibilities:

  • Act as a liaison between patients, caregivers, staff, and other stakeholders
  • Manage service schedules and communicate changes to workers
  • Handle intake information, enter data into systems, and produce reports
  • Respond to inquiries via phone and email
  • Perform general administrative tasks as needed
  • Qualifications:

  • Post-secondary education preferred, with at least one year of relevant experience
  • Strong interpersonal and customer service skills
  • Proficiency in Microsoft Office, particularly Word and Excel
  • Ability to work independently and as part of a team
  • Clear background check required
