They are currently seeking Team Coordinators to facilitate communication between patients, families, and staff, ensuring scheduling and administrative tasks are completed efficiently.

Key Responsibilities:

Act as a liaison between patients, caregivers, staff, and other stakeholders

Manage service schedules and communicate changes to staff

Input information into databases and produce reports as needed

Handle inbound calls and emails related to services

Perform general administrative duties

Qualifications:

Post-secondary education preferred with at least one year of relevant experience

Strong interpersonal and customer service skills

Proficiency in Microsoft Office, especially Word and Excel

Attention to detail and ability to learn new systems

Effective communication skills and ability to work non-traditional hours

Experience in healthcare field and clear background check preferred

Overview

CarePartners is adding to our team and we are looking for Team Coordinators to ensure effective customer service and communication between patients, families, and staff. Our Team Coordinators ensure the scheduling of our PSWs, Nurses, and Therapists meets service delivery standards and administrative functions related to client services are performed in a timely manner.

We have immediate PartTime openings in the Brampton office!

Hours of work will include evenings and weekends (2 pm  9 pm or 4 pm 9 pm)

PartTime roles include shifts every other weekend and there is an opportunity to pick up additional shifts.

 This position offers a competitive compensation grid, with progression based on hours worked and rolespecific criteria. The hourly range is $19.10 to $25.63, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate's knowledge, skills, qualifications, and relevant experience.

What The Role Involves

You'll be the bridge between patients/caregivers, their families, Personal Support Workers, team supervisors, funders, finance and other CarePartners employees
 Receiving routine referral/intake information from funders and privatepay clients
 Developing and coordinates service schedules while considering relevant factors
 Informing workers of new assignments and changes to their schedules
 Entering relevant information into the automated information system/database
 Producing schedules and reports on regular basis, and on request of the Supervisor
 Receiving and manage inbound telephone calls and email inquiries related to service
 Performing other administrative duties
 Other duties as required

What You Bring

A strong desire and commitment to making a difference in the lives of their patients
 Postsecondary education is preferred and one (1) year of jobrelated experience (such as administrative or customer service) is required
 Exceptional interpersonal and customer service skills
 Ability to work both independently and as part of a team
 Strong proficiency in all aspects of Microsoft Office, with proven skills in Word and Excel
 A strong attention to detail and the ability to learn new systems and databases
 Effective verbal and written communication skills
 Nontraditional hours, including evenings and weekends are required
 Experience in the health care field an asset
 A Clear background check
