This job posting is for a Wellness Navigator position located at Bedford Place Mall in Central Zone. The Navigator will support community members in accessing health services and programs, providing guidance, emotional wellness programming, and facilitating other health and wellness programs. Collaboration with the Community Health Team, family physicians, and community groups is essential. The role involves developing, implementing, and evaluating a community-based wellness navigation model.

The ideal candidate will have a Bachelor's degree in a health care discipline, registration with a professional regulatory association, and at least 3 years of related experience in mental health or primary health care settings. Strong facilitation/education skills, experience working with chronic conditions, and knowledge of health promotion strategies are required. Interpersonal, teamwork, and communication skills are essential, as well as computer proficiency and a valid driver's license.
